Serious Near Miss – Rail Car Door Fell Off
A rail car door was opened by the warehouse operator who was following the safe work procedure and using the lift truck attachment (steel frame). The operator fully opened the rail car door, backed the lift truck away and disconnect the steel frame attachment from the lift truck. The rail car door then fell onto the warehouse floor.
