Safety Alert
During a routine hazard assessment with three other millwrights, a decision was made to reposition all chains back one lug in accordance with standard procedure. After the equipment was properly locked out, an apprentice attempted to realign the chains by leaning in from the side rather than using the typical head-on approach.
