Details of Incident / Close Call: 

While constructing a pilot trail, half of the low side slope of the trail failed causing five cedar trees to fall. As a result the hoe was also shifted and left at a 35 degree angle. Fortunately, the machine operator was able to safely maneuver the hoe back to stable ground on the trail.

The immediate cause was said to be unstable ground.

Learnings and Suggestions: 

• Inspect worksites for alternate methods or routes in the event of changes to worksite conditions

• Review SOP’s for the job, especially stop work procedures and references to changing conditions
