Details of Incident / Close Call: 

A logging truck operator was holding pressure against the top of a logging truck trailer top stake – applying pressure to release holding chain to flip the stake up.

The worker’s hand slipped off the stake, releasing spring pressure – which resulted in the spring-loaded stake to hit the worker in the face. This resulted in a chipped front tooth injury to the worker.

Learnings and Suggestions: 
  • Equip logging truck trailers with a stake tripping pole / device that logging truck operators may incorporate to trip / unload stakes.
  • Ensure all logging truck operators are trained to position themselves in a safe place that will allow them to trip / unload logging truck trailer stakes without endangering themselves or other persons.
  • Ensure that all logging truck operators use a stake tripping / unloading pole / device to trip / unload logging truck trailer stakes.
