Worker twists leg on uneven ground at worksite. An excavator had been on site and left the worksite ground uneven. When working with a chop saw, the worker went to rotate the board he was working with for another cut. He stepped back to place the board and his left foot slipped into a hole 8-12 inches deep. This in turn jarred the workers right hip and knee. 

Learnings and Suggestions: 

Inform and ensure that Excavator is to leave the worksite area adequately groomed.

Workers are aware of their work area and inspect it before commencing work. The uneven ground was brought up in a previous toolbox meeting – Supervisor to ensure workers are actively participating and understanding issues communicated in the toolbox meetings.
