Company Name:
Delchako Timber Ltd.
Details of Incident / Close Call:

Heading back from taking a co-worker to the hospital (following a log truck accident) on a calcium Forest Service road at 11:30 am on a clear, sunny, Friday morning, a Level I First Aid Attendant was returning to work.  He fell asleep at the wheel, drove into ditch and flipped the crewcab pickup truck onto its roof.  The driver (who was wearing his seatbelt) was picked up by a log truck, met on route by the truck foreman and then taken to the hospital where he was treated for minor injuries and released.  The crewcab pick-up was demolished (non-repairable).

Learnings and Suggestions:

 ·          Discuss forms of Impairment - there are many (ie lack of sleep)

·          Maintain that ALL Workers are alert while on the job.

·          Review Safety Triangle with all Employees

·          Keep MIND ON TASK

·          Continue to Reinforce the use of Seatbelts

·          Watch out for Co-workers and take a Pro-active Approach should another Worker show signs of Impairment
