A worker was descending a steep bank approximately 12 feet high when the rock he was using for a hand hold suddenly dislodged causing him to fall forward landing on his arm dislocating his shoulder.


Learnings and Suggestions: 

Upon investigation it was noted that there was at least two much safer ways down the bank. The worker should have assessed the risk before choosing his route down and chosen the safest route.
