Momentary distraction leads to crew vehicle rollover

While travelling down a gravel road on the way home from work, the driver momentarily took her attention off the road and the truck got too close to the soft shoulder. She was unable to steer the truck back on the road and as a result rolled the truck in the ditch.

The truck rolled one and a quarter turns and came to rest driver’s side down, perpendicular to the road along the ditch bank. Two other crew vehicles were traveling behind and were not involved in the accident. Both vehicles stopped to help get people out of rolled truck. Everyone was wearing a seat belt and all walked away from the accident. The crew used the satellite phone to call the office and notify them of the situation. They then cleared the rolled truck of equipment and proceeded to town in the other vehicles.

Learnings and Suggestions: 

• The driver was distracted by reaching for a handheld radio in the truck (secondary to the truck radio used for road travel). A crew member from one of the other trucks was asking a question over this radio. The driver’s focus should have remained on the road and the copilot should have dealt with the second radio. Company drivers’ policies and procedures have been reviewed with the driver and an emphasis on the role of the co-pilot reinforced.

• Extra attention should be paid to changing road conditions. This accident occurred at a portion of the road where the running surface narrowed slightly due to the ditch dropping away. Thus there was less room for recovery when the truck tires encountered the soft, steeper road shoulder. A safety alert has been sent to all company drivers to alert them of this type scenario.
