The banding man was getting into the banding truck when it was struck and subsequently pushed 3 feet by a log that pivoted when the front left tire of the stacker working in close proximity made contact with the log (see attached pdf for diagrams of incident).

The worker banged his shoulder as he caught himself falling out of the cab.

Learnings and Suggestions: 

Learnings & Suggestions:

  • No loading out while in proximity to ground crews
  • Maintain a high level of communication with ground crews
  • Avoid staggering piles wherever possible. If piles must be staggered, create a "No Work" zone when ground crew is in the vicinity
  • Schedule work to reduce congestion
  • Ensure procedures exist and review Safe Work Procedures for relevant content
