Near Miss: Buncher Operator and Tree Planter too close together

A buncher operator was working along the boundary of the block when he saw something “brown” moving outside close to where he was bunching. He stopped working once he realized it was a tree planter working only about half of a tree length away from where he was bunching.

He was unaware that the planter was that close to him but stopped until the planter finally worked his way far enough away so he was no longer at risk.

Key Learnings:

This could have been a very serious incident, as the planter might have been severely injured had the buncher operator not noticed him when he did.

The planter was not wearing a Hi-Vis vest and the buncher operator was not aware of the radio channel that he could use to communicate with planters working in the adjacent block.


Learnings and Suggestions: 
  1. Whenever working around active equipment it is the responsibility of those on the ground to ensure they are at least two tree-lengths away and that they are the ones required to get the attention of the equipment operator before approaching.
  2. Planters need to wear Hi-Vis when working within close proximity of active, working equipment.
  3. If contractors are working in close proximity with one another - regardless of the type of work being done - the following should be discussed at the pre-work stage, prior to starting work:
  • Identify the radio channel to be used for communication with one another. This information needs to be communicated to all employees.
  • Provide the name of the person or supervisor that should be contacted if required.
  • Indicate the contractor who has been designated Prime Contractor, and what the responsibilities are what that designation.
