Switchback logging road gives way after rainfall

Safety Alert Type:
Log Hauling
Location:
near Christina Lake, BC
Date of Incident / Close Call:
2012-06-15
Company Name:
P & D Logging Ltd.
Details of Incident / Close Call:

A logging truck driver was attempting to negotiate a tight right-hand switchback that was made slippery by recent rainfall. The bottom, inside edge of the switchback had become saturated and unstable.

The tractor made it completely around the switchback but the trailer was still tracking to the inside of the switchback, too close to the over-saturated shoulder. This caused the road to give out. The trailer tipped over when the road collapsed, pulling the tractor over with it. Both units ended up in the ditch.

Damage to the rig is yet to be fully determined. Meantime, the driver strained his left arm snd scuffed two knuckles from holding onto the steering wheel. He might also have a mild concussion from when he released his seat belt and fell to the passenger side of the cab.

Learnings and Suggestions:

Corrective Action Taken: Hauling was stopped until the inside edge of the switchback was repaired.
