Serious Incident: Inspect wheel nuts often!

While travelling from Nanaimo to Gold River in the evening, the driver of a crew vehicle noticed a vibration coming from the rear of the vehicle when the brakes were applied. The driver stopped to investigate but nothing obvious was wrong.

The crew continued travelling and as they did, the vibration worsened. The driver again decided to check the vehicle after stopping in Buckley Bay. At this time the driver noticed the left rear wheel had only two lug nuts remaining out of five and the wheel was almost off the vehicle.

Learnings and Suggestions: 

The vehicle was taken to a licenced mechanic at the dealership where it was purchased from. No other wheel was affected and it had been almost 15,000 kilometres since the wheels were last removed.

Findings: After inspecting the wheel in question, it was determined that the wheel had been tampered with.
