Details of Incident / Close Call: 

An RW Log Transport employee was fueling up his logging truck. When he finished, he went to put the cap back on the truck’s fuel tank before hanging up the nozzle. This completed the circuit, causing an electrical current to pass through the employee’s body and resulting in immediate muscle spasms, numbness, tingling and considerable swelling on his right arm and hand. He also suffered two exit holes in his right hand.

This incident might have been avoided, as it has been reported that the site supervisor was aware of this issue about three weeks prior to this occurrence.

The cause was determined to be wiring on the fuel pump that was not properly grounded.

Learnings and Suggestions: 

Full and complete repair to the electrical fuel pump wiring.
