Excavator slides off sub-grade, saturated soil suspected cause

Excavator operator was building sub-grade in endhaul, skirting around rock outcrop, placing stumps and logs as supports. Excavator travelled over the area, continued working forward and at the end of the day, travelled over the area again going out to fuel the machine.

The next morning, the excavator travelled over the area a third time. When work commenced, the material gave way and the excavator slid down the bank and came to rest against right-of-way logs.

Learnings and Suggestions: 
  • Stop sub-grade work until rock outcrop work is blasted
  • Refrain from skirting around rock outcrops - wait for drill/blasting
  • Site conditions must be continually monitored by the operator, foreman and engineer
