Hauling in winter conditions, a loaded logging truck left the landing and travelled approximately 300m down the logging road when the truck slipped into the inside ditch. The momentum of the trailer caused both the truck and trailer to slide and flip onto their sides causing damage to the unit.

An investigation showed the main cause of the incident to be changing road conditions due to weather. The temperature was around 0 degrees making road conditions very slippery. There were also several recent heavy snow events in the area causing a build up of snow on the logging road. It was found that the road was over plowed toward the inside ditch and the truck driver had inadequate chains at the time of the incident. 

 

Learnings and Suggestions: 
  • All employees need to communicate changing site conditions and the need for maintenance or repair immediately (especially after a heavy snowfall event).
  • Grader operators need to be aware of road slopes and ensure adequate plowing is completed to allow the movement of traffic safely away from potentially hazardous areas.
  • Truck drivers need to ensure they are using adequate chains when conditions require it.
