Beware of the Bounce!

A log truck driver was at the deband station at Polar Saw Mill at Bear Lake, BC when he placed the deband arms into position for removing the wrappers. When the arms were flipping out, the momentum caused the arms to bounce. The driver grabbed the deband arm handle to stop the bouncing but the momentum of the bounce pinched the driver’s finger between the handle and the roof of the deband station. The driver received first aid attention and was required to go to the hospital for stitches.
 
Root Cause:
 
An improper attempt to save time was the root cause of this incident. The truck driver should have either taken the time to place the deband arms in place instead of “pushing over centre”, or he should have waited for the arms to stop bouncing. As the driver was in a hurry, he misjudged the force and weight of the deband arms.

 

Learnings and Suggestions: 

Review the incident with drivers who use the deband station and issue a Hazard Alert emphasizing slowing down and paying attention to the task at hand.
