Stacker Operator drove into the weigh bunk to lift a bundle after being weighed. As he lifted the bundle up

over the bunk stakes and high enough (approx. 14 feet) to clear an adjacent loose unweighed log pile, logs fell

out of the bundle falling towards the weigh station office. Approx. 6 logs fell out, which slid towards the office

with one log coming very close to hitting the office (with the weigh master inside). Of particular note is that

the log entered into the area previously thought to be a “safe” area.

 

Learnings and Suggestions: 


It is recommended that similar sites across the industry take a second look at areas previously thought

to be “safe” and to re-analyze them based on current equipment and working conditions.


Ensure Safe Work Procedures identify the standards to be adhered to when working in close proximity

logs were lifted likely contributed to the distance the log traveled when it hit the ground.

Ensure measures are taken to minimize the height that logs are lifted. In this instance, the height the
