Supervisor was driving from Gold River to Campbell River. Road conditions were good with typical night-time driving conditions. Front driver’s side wheel detached from vehicle at highway speed. Driver was able to stop the truck safely. Inspection revealed that all lug nuts on affected side were missing – in addition the three remaining wheels all had loose or missing lug nuts. The truck’s wheels had been recently been removed / reinstalled as part of a brake system service..It appears that lug nuts were not correctly re-torqued to specifications following the service.

Learnings and Suggestions: 

 

Check that wheel lug nuts are torqued to manufacturer’s specifications any time wheels are removed and at weekly intervals thereafter.
