The haul road for a new worksite was built in 1960’s. Some improvements had been made in the 1990’s but it is still had several design/construction problems that made use by modern configurations difficult. (Steep, narrow, tight switchbacks)

The contract supervisor recognized the potential problems associated with the road during the pre-work and initiated improvements to mitigate the hazard.
After initial use further problem areas were identified.

The lead trucker conferred with the supervisor and safety coordinator. A more detailed road evaluation was conducted and a set of road use procedures developed including shut down criteria.

Learnings and Suggestions: 

Contract supervisors should utilize the Forest Safety Council Road Assessment Checklist as part of their pre-work for roads.  Information on the form should be communicated with all road users prior to use and acknowledgement or receipt of the information documented.

A detailed set of design/construction standards required for current truck configurations should be developed.  Truck Safe might be the best agency to conduct this.
