2009-10-09 Fairlead breaks off grapple yarder

Details of Incident / Close Call: 
  • During operations with a Madill 123 grapple yarder, the plate and bolts securing the top pivot of the guyline fairlead broke off.  The fairlead fell on the guylines to the railing at the back of the machine.
  • It was revealed that the plate was cracked prior to the actual separation.
  • There were no injuries but two guylines required replacement and the repair costs were significant.
  • There was potential to cut all three guylines and cause a roll over situation.
  • Subsequent discussions with others show that this failure has happened at least three times with other similar machines

 

Learnings and Suggestions: 
  • Regular inspections of the boom and gantry steel fabrications with special attention to this area when the gantry is lowered for moving.
  • The crack was not easily observed with the gantry raised and inspections must be done carefully.
  • The cracked plate was replaced with additional reinforcement.
