Description:

 

  • Employee stepped on a log with loose cedar bark and his foot slipped causing employee to fall into water. As employee fell into water he hit the right side of his head. Life vest the employee was wearing failed to inflate.
  • Upon inspection the manual inflate cord was pulled discharging the co2 cartridge and inflating the life vest. It is suspected the auto inflate valve was faulty.
  • Vendor has been contacted and the vest is being sent out for inspection of the valve. Findings of this inspection will be distributed to operations when it becomes available.

 

Learnings and Suggestions: 
  • Inspect autoinflate valve to ensure the marking is showing green. Red indicates previous discharge and shoud not be re-used until adequate inspection and re-charge is completed.
  • Ensure co2 cartridge is not spent and that it has not become loose or detached from valve.
