2009-10-26 Employee struck in safety glasses by air hose

While setting down a pike pole it struck an air hose coupling causing the male fitting to separate. The gate valve was not shut off and the residual air pressure caused the end of the hose to violently swing towards the employees face striking him in the safety glasses.

Learnings and Suggestions: 


Ensure air pressure is bled before uncoupling an air hose fitting or when air hose is not in use.
Avoid leaning tools or other equipment against couplings.
