2009-05-05 Planter slips on log - bruises ribs

Details of Incident: Worker fell while walking on a log on a tree planting worksite, landing broadside across the log. Worker was using proper PPE including caulk boots with caulks in good condition. Worker sustained bruised ribs and required a few days of recovery before returning to work.

Learnings and Suggestions: 

Exercise caution and sure-footedness at all times in work settings. Stress continued proper use of PPE and proper condition of PPE. Highlight these issues in safety meetings.
