2009-08-12 Driver fatigue suspected in crash

Details of Incident / Close Call: 

A logging truck driver was empty heading to the bush for his first load of the season. At 2013km he crossed the road and hit the ditch on the other side of the road. He travelled in the ditch for approximately 180 meters before coming to a stop. The driver was badly injured and there was significant damage to the truck.


Root Cause(s):
At this time the root cause appears to be driver fatigue.


Learnings and Suggestions: 


- Ensure that all truck drivers, pickup drivers and employees in general know the importance of a "good nights sleep".
- If you are taking medications, it is your responsibility to be aware of any possible side effects. For example, "Taking this medication may cause drowsiness".
- If you are not feeling "yourself", stop work immediately and notify your supervisor of this information.
- If at any time during the day you feel fatigued, pull over and stop when it is safe to do so.
