On July 29, 2009 a pick-up truck rear-ended a loaded hayrack log truck while attempting to overtake the loaded truck in very dusty road conditions. The pick-up driver had radioed ahead to the loaded log truck and received instruction from the log truck driver that it was all clear to pass and that he would slow right down to allow the pass. A combination of bright sunshine and very dusty road conditions did not allow the pick-up driver to see the back of the loaded log truck in time to avoid a collision.

No injuries resulted from this incident, but the pick-up sustained approximately $9,000 damage. However, this incident had the potential to cause a serious injury or fatality.

The investigation revealed that the pick-up driver failed to adequately assess the hazard created by the sunshine and dust cloud, and was lacking situational awareness by allowing himself to get too close to the loaded log truck and resulting dust cloud.

Learnings and Suggestions: 

ACTION: The pick-up driver reviewed the company SOP for driving and rules of the road, an incident investigation was conducted and shared with the rest of the company employees, and a Hazard Alert was developed for internal and external posting

 

RECOMENDATIONS
1) All drivers need to recognize the visual impairment hazard that dusty road conditions and bright sunshine create, and not allow themselves to get caught up / blinded in the dust cloud of a lead vehicle.
2) Do not attempt to overtake / pass a vehicle unless you can clearly see the roadway and vehicle ahead.
