2009-07-17 Lockout fails - skidder cab falls on mechanic

A mechanic was raising the cab on a processor to do maintenance when the cab went over center. It then free fell and came to an abrupt stop on the emergency cable. Upon further investigation it was found that the cotter pin had come out of the cab cylinder.

 

Root Cause(s):

- Inadequate or lack of inspection of the pin in the cylinder. - The cotter pin had worked itself free and was no longer in the cab cylinder.

 

Learnings and Suggestions: 

 

- Ensure that pin is in cylinder before attempting to raise the cab on any piece of equipment!
- All other equipment was inspected the same day to ensure that the pin was in place.
- Always be aware of your surroundings and the people who are working in your area.
