Worker was climbing up steep stream bank through thick riparian vegetation and was struck on cheek below left eye with the broken end of a limb that was concealed in the brush.  Resulting injury required first aid only.  No time loss or medical aid.

Immediate Cause(s):

Abrupt climbing motion while hanging on to brush.  No hand free to ensure path was clear.

Urgency to complete task.

Basic Cause(s):

Inadequate hazard assessment along route of travel.

Root Cause(s):

Inadequate compliance with standards. -hazard assessment incomplete.

Learnings and Suggestions: 

Take time to pass through thick vegetation safely as hazards are concealed.

Keep head tipped down so hard hat is the first object to contact anything.

Discuss wearing safety glasses in bush at the next safety meeting.
