2009-07-26 Millwright struck in neck by foreign object

Contractor weekend Millwright was asked to assist other millwright on Reducer Quad Step-feeder task. This task involved some welding and grinding, and the helper millwright was getting his 4 1/2" angle grinder ready for task. The angle grinder required a new grinding disc. To change the disc, the helper millwright used manufacturer wrench and grinder arbor lock mechanism to try to break free disc lock nut. He could not break free lock nut. The arbor lock mechanism was slipping and so he requested the help of millwright on step feeder task. The two of them went to maintenance shop and decided that the helper millwright would hold on to old grinding disk, while the other millwright, using a hammer and pin punch would strike nut and try to break it free.

This process was repeated about 2 times, on the third strike with hammer on pin punch, the helper millwright felt something hit him in the throat. The other millwright saw blood coming from area on throat and immediately pressed the first aid alert button in shop area. Millwright was taken to hospital to have foreign body removed from throat requiring 3 stitches.


Learnings and Suggestions: 
  • Recognize Risk: Ensure you have inspected tools and are wearing appropriate PPE (Face Shield, Eye Protection).
  • Please review the hazard potential of using hammers, drift pins, etc during crew talks.
