Driver was driving along a flat portion of the logging road when the pintle hitch broke causing the trailer to leave the roadway pulling the truck onto it’s side.
During the investigation, it was found that the driver had forgot to secure the safety chains.
It is not clear if this would have prevented the accident from occurring, but definitely would have helped.
 

Learnings and Suggestions: 

Inspect hitch mechanism more thoroughly and frequently.
Ensure safety chains are correctly secured at all times.
Daily truck inspection as per pre-trip requirements.
