2009-06-23 Knives fall from carrier

Knives were sitting along the edge of a raised platform which had no toe kick in place. Mill vibrations caused the knife carrier to work its way over the edge falling to the area below in front of an entrance/exit to the mill. 

Knives were left unsecured near the edge of a raised platform. 

No toe kick on the raised platform.

Learnings and Suggestions: 
Ensure knives and carrier are not stored or left unattended in hoisting area.
Install toe kick on raised platform.

Paint area below to identify overhead hazard.
