Safety Alert Type:
Manufacturing
Location:
Western Forest Products Site APD
Date of Incident / Close Call:
2009-06-01
Company Name:
Western Forest Products
Details of Incident / Close Call:

Hot work was being performed over the weekend in the area directly above the grinding room. As a result water from the wetting down process leaked through the floor boards down into the grinding room area getting into the collar on the Babbitt ladle.

The following Monday the area in the grinding room was dry however the water which had gotten into the collar of the ladle was still present. As the grinder man went to scoop out some Babbitt the water ran out of the collar of the ladle and into the Babbitt pot causing it to explode out towards the employees face. The employee was wearing all the required PPE (Face shield, safety glasses, heat resistant apron and gloves) consequently preventing serious burns from occurring.

Please take time to review this incident to help reinforce the importance of wearing necessary PPE. This incident would most likely have caused serious injury had good safe work practices been ignored.

Learnings and Suggestions:
  • Review importance of wearing all required PPE.

  • Inspect area for hazards such as water or other persons in or around the area who may be in the line of fire.
