A worker was backing an ATV out of the warehouse; the
worker was wearing a helmet. Once outside of the warehouse while still rolling in mid turn with reverse still engaged, the worker leaned forward to inspect the front tire. The worker’s thigh touched the throttle causing the ATV to jerk backward throwing the weight of the worker onto the throttle even more and the worker further off balance. The ATV sped in reverse following a semi circle path, for a time balancing on two tires. After about 10 feet the ATV met a concrete wall sending the rear tires spinning up the wall. The ATV was vertical against the wall the front two tires on the ground. The worker fell forward off the ATV and rolled forward. The worker was helped to safety by one witness, while the quad fell onto its side and was managed by two other witnesses. Only minor scrapes resulted.

Learnings and Suggestions: 

Ensure that the ATV is off and has the parking brake engaged during inspection. Also, ensure that when the transmission is in gear full attention is on driving and not focused on other tasks.
