2009-04-01 Wheel falls off service truck

A heavy equipment mechanic was driving one of the company’s service trucks while conducting a service call. To access the excavator requiring the service call, the service truck was driven on a rough, bumpy forest road near Rossland, BC.

The mechanic completed the service call and was driving the vehicle on Highway 6 near Slocan Park (enroute to Nakusp, BC) when the right rear wheel suddenly fell off the truck.

The mechanic managed to stop the truck without incident but had to call for assistance because the service truck was not driveable following the incident.

Learnings and Suggestions: 
  1. Vehicle operators must conduct documented pre-trip inspections of the vehicles they are operating on a per use basis. This means that each day the vehicle is operated, the operator must conduct a documented pre-trip inspection.

  2. Ensure all vehicle wheel lug nuts are checked for tightness on a daily, per use basis. This is particularly important when the vehicle is being drive on rough, bumpy roads.
