On December 10, 2008 while one of our supervisors were coming back from a worksite driving on Highway 16 the driver at this point was descending down a small hill when his truck hit some black ice. The pickup spun at a 360 degree turn and the front tire of the truck hit a barricade swinging the truck out of control.   

INVESTIGATION: It was noted that the driver of this vehicle had been up early and had been driving while fatigued. The pickup was a complete loss (see pictures)

Learnings and Suggestions: 

All drivers must be prepared for the unexpected at all times especially when we are dealing with winter conditions. Never use the cruise control while driving in winter conditions. Do not drive if you are fatigued; fatigue can slow down the brains trigger response. Pull over and have a 10 minute rest and/or walk around your vehicle before commencing the trip.
