2009-03-05 Tow Line Breaks Sends Trailer Over Bank

Location:
Car Wash Rock Hwy. 16 Terrace
Date of Incident / Close Call:
2009-03-05
Company Name:
Cross Creek Logging Ltd.
Details of Incident / Close Call:

Empty logging truck was towing his empty tridem trailer back from Prince Rupert when the trailer became unlatched and went over the bank.

Upon investigation it was noticed that the safety chains were not connected properly and the reach lock had not been tightened . The latch that holds the insert had bounced up along the way back and the trailer insert slid out of the main reach tearing the brake lines and electrical cord . The trailer then went over the bank.

Learnings and Suggestions:

The driver must ensure when he is towing his trailer that the reach lock is tightened and the safety chains are done up properly. It is also suggested that the latch that flips over the sliding insert is inspected so the springs that hold pressure on the latch are in good working order. It is also suggested that a tie down of some type is put around this latch so that it cannot bounce up along the road.
