The buncherman had finished his shift and was cleaning branches off his cab; he was standing on the boom base. When he was done he stepped down onto the fuel tank, slipped on some oil that was on it and fell on the handle of the fuel tank. The buncherman fell on his lower right side of his back, causing massive bruising and possibly 2 cracked ribs. The buncherman was in a lot of pain so a crewmember drove him to the hospital.

Root cause: Improper 3-point dismount. Oil on fuel tank.


Learnings and Suggestions: 
  • Do proper 3-point dismount.
  • Be aware of surroundings. (oil on fuel tank)
  • Keep fuel tank free of oil.
