2009-02-27 Hook tender escapes injury

122 Grapple Yarder was walking past 044 Grapple Yarder in a turnout when the track sunk. The operators hooked lines from one GY to the other in order to assist the 122 back onto solid road surface. One hook tender had moved into the bight to notch the stump thinking he had time. The hook tender had communicated his intentions to the operator of 122, but the conversation wasn’t confirmed by either worker. When the 044 operator pulled on 122 GY the standing tail holds were pulled over at the backend and narrowly missed hitting the hook tender. 

Learnings and Suggestions: 
  • Insure that the ground is solid enough to hold up a machine before committing the weight of the machine.
  • Use guy lines when required.
  • All tail-holds must be assessed prior to imposing extreme stresses on them.
  • Develop a plan and communicate that plan with all parties involved.
