2009-02-19 Operator slips while changing oil

Processor operator was standing on catwalk of his machine, changing oil. He lost his footing and slipped. He grabbed onto exhaust pipe and grab handle, but fell onto catwalk. Oil had spilled onto the catwalk earlier, making it very slippery. Luckily, minor injuries only.

Learnings and Suggestions: 

Operator should have cleaned up the oil before attempting to walk on the catwalk. As well, the operator should have employed a proper three point stance. Address issue at tailgate/safety meeting to discuss the importance of using the three point stance, and keeping surfaces clean of oil, etc.
