Details of Incident / Close Call: 
  • Driver was traveling loaded down the road.
  • Chains were on the truck, but not on the trailer.
  • The trailer slid into the ditch.
  • The road pitch steepened.
  • The driver tried to stop, and looked in the mirror. The front tires of the trailer were in the ditch.
  • Truck and trailer slid @ 200 ft. down the road.
  • The trailer stake hit rock and both units stopped.
  • The truck then rolled gently on the driver’s side.
  • The road where the trailer first went in was very icy.
  • The road was not sanded properly.
  • This incident resulted in no injuries, but the potential for injury could have been great.
  • The incident resulted in minimal truck damage.
  • The direct cause for this incident was icy roads, chains only on the truck and the driver’s lack of experience in winter driving conditions.

 

Learnings and Suggestions: 
  •  Better screening of driver’s experience, particularly in winter conditions.
  •  Do not haul on any road when it is not properly maintained.
