On December 2, 2008 a trailer stake broke away from the bunk of the trailer while the loaded log truck was making a right hand turn. The gusset on the front left bunk failed causing part of the load to spill into the oncoming lane. No other vehicles or pedestrians were involved.

The investigation determined that the trailer stake broke at the point where the base of the stake had been recently welded to the bunk. While the welding was done by a certified welder, the design and type of welding was inadequate for the stresses experienced by the gusset.

Learnings and Suggestions: 
  • Any welding repair or modification to an engineered component is required to be fabricated according to a design specification from a professional engineer or the manufacturer.
  • Templates for stakes and gussets are readily available from manufacturers.
