On November 20, 2008, one of the sub-contractor’s processors was moving across a road towards a new deck.  As he crossed the ditch the machine fell into a burrow pit that was made when main haul road was built last winter.  The operator did not see the hole as it was full with water and had frozen over, with debris over top.


 

Learnings and Suggestions: 
  1. Road construction crew should ribbon or stake off potential soft spots such as burrow pits.
  2. Maps should have these areas highlighted so they can be brought to everyone’s attention.  Machine operators should be aware of potential risks and avoid them.
