Documents should be photocopies, not originals
Some documentation can be used for more than one question. If this is the case please clearly note for each relevant question where this document is, or copy it more than once
All documentation should be current and within the previous 12 months only
Completed Worker Interview Contact List (please inform all employees if they have been listed)
