Description of Incident
May 8 Yarding/Mechanical Harvesting I The operator of a grapple yarder attempted to level the yarder on the road using logs under the tracks.  During the process of climbing up on the logs with the tracks the grapple yarder lost balance and flopped over.  The operator suffered minor injuries. 
April 29 Log Hauling I A loaded log transporter was proceeding west along Trans Canada Highway #1 at or near Bridal Falls when the transporter encountered the scene of a traffic accident that had occurred approximately 20 minutes previously.  The operator of the log transporter attempted to stop and as a result of this action crashed into the cement barriers along side of the road.  Minor injuries were sustained by the operator of the transporter.  Significant injuries were sustained by 3 members of the public in a mini van that was involved in the previous accident. The transporter remained upright with the load intact. 
April 25 Log Hauling I A worker on a self loading log truck fell 11 feet to grade while attempting to change out a hydraulic hose located directly behind the operators seat. 
April 24 Yarding/Mechanized Harvesting SI A worker was dismantling the apex of a grapple yarder that was being salvaged when a section fell over and catching the worker's hand.
April 17 Manual Tree Falling F A certified faller was clearing debris from around the base of a small hemlock to fall the tree.  He completed the final cut of the ground debris (fir) when previously bucked cedar/hemlock trees with attached rootwad system slid down the steep hillside pinning the faller against the standing hemlock tree.
April 14 Manual Tree Falling I A worker engaged in spiral pruning (removing 30 - 70% of branches in tree crown) fell 51' from a Douglas fir tree ( 16" diameter by 100' tall ) to the forest floor.
April 8 Other I A worker was attempting to buck a 38' x 18" diameter cedar log (approximately 2400 lb) that became dislodged during transport and was suspended in the cinches of a logging truck.  When the worker finished the bucking cut, one section of the bucked log swung under the load and pinned the worker against the reach of the logging truck.
April 3 Log Hauling SI A loaded log transporter, jeep with a wagon, rolled over while negotiating a curve.  The entry to the curve was measured at six percent favorable. The truck rolled at the mid point of the 90 degree curve.
March 31 Log Hauling F A loaded logging truck was traveling south bound on the Alaska Highway.  The logging truck was travelling down hill and had entered into a left hand corner.  The truck failed to negotiate the corner with the trailer flopping over on its side while still on the highway and then coming to stop in a ditch on the side of the road. The driver  was thrown around in the cab of the truck and suffered fatal injuries.
March 26 Manual Tree Falling SI A certified manual faller was attempting to fall a small hemlock tree, 6 inch diameter by 67 feet tall, when it hung up in a standing tree in front.  As the faller was walking under the hung up small tree to reach the base of the standing tree in order to fall both trees together, the small tree dislodged and hit the faller. 
March 26 Traveling to and from Work I Two workers were traveling in a pick up truck on a forest service road.  The driver lost control of the vehicle and the vehicle rolled over.  The road surface at the time of the accident was covered with 1 centimeter of fresh snow.
March 16 Manual Tree Falling I A certified manual faller was felling a dangerous tree that hit a previously felled tree.  The previously felled tree bounced and flipped the faller into the air.  The faller sustained injuries to lower extremities.
March 10 Other SI A worker received injuries resulting from a paint can exploding in a debris fire, striking the worker in the upper area of the body. 
Feb 26 Yarding/Mechanical Harvesting I Working on a steep slope during cable operation, a hooktender slipped on the snow pack, resulting in a piece of logging debris  (stick) puncturing and tearing off a portion of flesh on the workers upper extremity.
Feb 12 Manual Tree Falling F A certified faller was struck by the top of a dangerous tree approximately 138 feet tall.  The dangerous tree had been cut up and pushed over by another tree to overcome a falling difficulty by the workers falling partner.
Feb 12 Other SI A worker was using a power saw to rip a fir log, which was 4.3 meters long and 1.5 meters in diameter.  The worker lost  footing and fell under the log slabs, when the ripping cut was completed and the log slabs shifted.
Feb 11 Log Hauling I An 8-axle loaded log truck left the loading area to commence the trip to a sawmill.  While slowly transversing a left leaning bend in the road the back wheels of the trailer went over a small bank pulling the trailer and truck unto its drivers side.
Jan 25 Manual Tree Falling SI A certified hand faller sustained serious injuries when the worker was struck on the back of the head by a limb.
Jan 22 Manual Tree Falling F A cedar windfall that was being bucked by a certified manual tree faller pivoted, pinning the faller up against a rock bluff.
Jan 16 Log Hauling F, I The driver of a loaded logging truck was fatally injured when his truck collided with an empty logging truck.  The driver of the empty truck lost control when he hit his brakes and slid into the path of the loaded logging truck.  This accident occurred on a public highway.

April 22 Veneer or Plywood Manufacture SI Worker was descending a set of stairs holding the rail and turned to assess noise from the chipper.  Upon turning back to the descent, the worker thought that the next step was the floor, however, it was the second to last step, and the worker caught their heel and pitched forward falling to the floor.
April 14 Sawmills I While working at a shingle machine a sawyer lost his footing and as he reached out to steady himself he placed his hand on the clipper saw board which depressed allowing the energized clipper saw to contact his hand. 
April 10 Sawmills I A worker was assisting co-workers to straighten and adjust the vertical wood splitter on a portable sawmill when the worker’s finger came into contact with a rotating circular saw. 
March 20 Oriented Strand Board Manufacture 2I Workers employed at a Strand Board facility were working from a man lift unit cleaning materials in the ceiling area above the board press when they were exposed to a flash fire and smoke inhalation.  These workers were also scalded from a hot water discharge from the fire suppression system.
March 13 Shake and Shingle Mill SI A worker with 8 years experience as a shingle sawyer suffered a serious injury when the worker came into contact with an energized clipper (trim) saw on a Sumner shingle machine.
March 5 Sawmills SI A millwright reached through a gear and sprocket drive on a wood waste conveying and screening system (hog disc scalper) to grease a bearing.  The equipment was not locked out.  An apprentice millwright started the equipment resulting in serious injury to the millwright.
Feb 28 Sawmills SI At about 02:00 AM the worker appears to have attempted to clear a trim block box under a trim saw in the sawmill.  The worker’s lower extremities contacted the trim saw causing serious injuries.
Feb 22 Shake and Shingle Mill SI A sawyer's was seriously injured by a shingle machine circular saw when it came off the arbour at full speed.
Feb 21 Sawmills I A worker was clearing crossed boards at the in feed area of a planer machine when the worker was caught between the board and an energized and rotating 'pineapple' roll.
Feb 4 Sawmills SI A worker was attempting to straighten a skewed 9-foot log in the in feed unscrambler by hand when the log suddenly straightened.  The workers hand was caught between the handrail and the log. 
Jan 26 Sawmills SI A worker using a "zip-cut" type metal cutting wheel on a small hand held grinder with the guard removed. The wheel shattered causing serious injuries.
Jan 23 Sawmills SI The sorter operator received serious injury when the worker reached in to adjust a skewed board.  The machine was not locked out.
Jan 17 Sawmills I A worker was operating the J-Bar Unscramble when the worker reached across the moving ending rolls to grab a board with a jagged end. The worker's hand was caught on the end of the board and was pushed up against an even ending side fence where the worker got caught in the in-running nip-point of the side fence belt conveyor and safeguarding.
Jan 17 Chemical Pulp and Paper Mill I A utility labourer who was working alone at the time of the accident was found caught between the travelling arm of the Head Master automated equipment and the walkway in a pit.
Jan 14 Sawmills SI A young worker was performing work activity in the area underneath a lumber stacker when the worker's hand was caught in the in-running nip point of a transfer chain and sprocket that were not de-energized.
