The machine slid sideways off the rock for approximately 4 meters. The tracks struck a stump causing the Buncher to flip over and roll three times down hill for approximately 30 meters until coming to rest on a 35% slope. |

The worker was able to grab onto a log from the boom but was unable to escape and remained in the water for approximately 10 - 20 minutes. A southbound loaded logging truck (tractor/single-axle jeep/ tandem pole trailer configuration) flipped onto its driver's side while negotiating a corner. Spilled logs swiped a northbound pickup that drove onto the shoulder, and some logs struck a second vehicle with two occupants. One log entered through the car's windshield. Air Ambulance transported the female non-worker driving the car to Vancouver General Hospital with fractures (arm and leg) after a 2-hr extraction. The second occupant (male non-worker) was in considerable pain and his injuries are unknown.
The logging truck driver suffered minor injuries and was assessed by paramedics on the scene and released. |

A worker was operating a grapple skidder on a moderately sloped harvesting block. A bunched pile of logs was sitting at the apex of a short hill (i.e. approx. 100ft. at 41% -45% slope). The operator tried to back up (i.e. upslope), to the butts of the logs to be skidded but the skidder failed to negotiate the slope. So, the operator drove around the hill and back up to the top where he drove over the log pile with his front left hand tire in the down slope direction to better position the skidder to pick up and drag the log pile. As the front tire dropped off the end of the pile and onto the excessive slope, the skidder became unstable and rolled 1 1/2 times down slope. |

A faller entered a clutch of 13 trees to fall an approximately 120 foot high fir tree with a 22 inch wide butt. At approximately 90 feet up, the fir was tangled by branches with a popular tree. The felled fir caused the popular to fall towards the faller striking another tree. This third tree snapped in the middle (i.e. jack knifed) and a piece of the jack knife obstructed the fellers escape route striking him on the back of the neck causing serious injury. |

Two hemlocks were growing out of a 6 ft-high, 4.5-ft diameter, old-growth cedar stump. The faller climbed on top of the old cedar stump and between the two hemlocks. Evidence suggests that the faller attempted to fall the smaller hemlock and it failed to fall due to being limb-tied with the larger hemlock (unknown whether wedge set at this time). The faller proceeded to fall the larger, hemlock in a direction opposite the intended direction of the smaller hemlock. The faller was apparently crushed by the smaller hemlock as he attempted to escape. Possible, contributing causes are being assessed. |
